Well, first of all, you need a server (computer). And you need the Ventrilo server program (which is separate from the client software.) The server program is free to download, but it's limited to only 8 users.
Unlike some games (for example, Half Life), where you can run the game as a "listenserver" where you can actually play on the "server" you are running, Ventrilo has a separate server program which you cannot also be "in" at the same time.

I'm pretty sure I tried running the Vent server program and the client on the same machine (quite a while ago) and it didn't work. Although I'm not saying it can't be done.

At any rate, if you have a spare computer around - it doesn't have to be much, an old pentium with Windows, or you can even install Linux - simply run the server .exe program to install it. If it comes up with a choice to run the server as a "service", do that. Then of course, you need to connect the server (computer) and your computer through a router to connect them both to the internet. You'll need to forward the appropriate port (3874 is default) to the server in the router settings. It's best to set the server up with a fixed IP rather than just let it get one from the router, so the IP doesn't change, but as long as you are willing to change port settings when needed, it doesn't have to be fixed.
Externally, people from the internet will use your IP (the one you get from your ISP) to connect to your Ventrilo. (The external IP can be found, usually, on the status page of the router setup.) Internally, you would use the network IP of the ventrilo server to connect. (usually something like 192.168.1.xxx)

I know this may seem like a lot of stuff to go through, especially if you're not familiar with IP settings, etc., but no one said it would be easy.
